This took place in Warsaw in the Parish of St. Alexander on March 10/22, 1895 at 10 a.m. Came before me Marcin Szulc, a brewer and Wladyslaw Tomaszewski, administrator of his hotel, residing the first in Brest Litovski and the second in Warsaw. They declared that yesterday (March 21, 1895) at 3 p.m. in Warsaw in house number 1713 B died Wladyslaw Wincenty (two first names) Praski, married, a varnisher, 50 years old, born in the town of Czestochowa, son of Ignacy and Antonina, legally married couple. He was survived by his wife, Zofia, maiden name Szulc (spelled Szultz). After verifying the death of Praski, this document was read to the reporting persons and signed by them and by me.
